Application Forms

Please note that in order to view the FORMS listed on the left, you will need a program which can open documents of type PDF, such as PDFXchange Viewer, Adobe Acrobat or FoxIT Pro. If you do not have such a program there are links on the COURSES page to online websites for suitable downloads.

Application checklist:

• PTG1 form
• DSA
• Medical note signed from your doctor stating your disability. This may incur a fee of £25, however this will be reimbursed.
• Proof of income - P60
• Once you have chosen your course all we have to do is to enrol you at the university and you are ready to start.
